Adding new forms to the Formbank™

The Formbank™ contains hundreds of payor forms from both national and state specific organizations, including the CMS 855 series and most state Medicaid forms. New forms are continuously being added to the Formbank™.

To submit a new form, send one of the following:

1. Download a PDF copy of the form or scan the form and save as a PDF. Send via email to support@medkinetics.com.
2. Send a clean, unaltered (original) form by mail to Medkinetics. Check with the payor to make sure it is the most current version available.

Medkinetics staff will map each blank with a field from the provider profile, so your physicians' data will automatically populate the form. NOTE: Some fields that are unique to a particular form will be mapped with a generic field that allows you to manually enter data in the context of that form.
